Position Summary
This post supports the hospital to attain it’s 95, 95,95 targets through implementation of linkages, adherence and retention strategies.

Job Description
Support linkages, Adherence and retention strategies at site level; Conduct training for HCW on Operation Triple Zero; Training of caregivers on the programme (I day training at site level); Identification and rolling in of AYPLHIV into the programme; Training of AYPLHIV on the OTZ in areas such as treatment literacy, understanding body changes as a result of HIV, ARVs and desired ART Goals; Form Clubs for AYPLHIV, designing OTZ Logos, motto and pledge into local language; Roll out the tool for screening mental health for all clients on ART; Facilities identify clients with advanced disease and create a list; Facilitate home visits for ART delivery, Viral load Monitoring and Enhanced Adherence Counselling for clients with advanced HIV disease; Support community dialogues for P&L women; Support community dialogues for P&L women in communities with high new HIV infections in children; Train and support Peer Adherence and Retention Facilitators and Young Mentor Mothers (YMM) in selected key entry points for the administration of screening tools in low yield entry points; Mentor the YMM and Adherence and Retention Facilitators in psychosocial counselling skills necessary to conduct successful sexual partner elicitation, Intimate partner violence (IPV) and gender-based violence (GBV); Provide psychosocial counselling services to patients in DSD and on MMD; In collaboration with the site HIV focal person, nurse counsellors generate a list of daily appointments, DSD and list of patients on MMD; The retention prioritization shall be based on the following criteria; Newly identified treatment naïve patients; Patients with unsuppressed viral loads (VL above 1000 copies/mL); Recently ART initiated (below 3 months) patients; Patients on DSD, MDD and on TPT; Work with nurse counsellors and VIAC nurses, Adherence and Retention Facilitators and YMM to: Follow all contacts from all index clients as per provided list so that they come to the facility for testing; follow up positive patients from the community and linking all to Treatment, Care and Support services; Work with the District Nurse coordinator to develop work schedules and performance enhancement interventions for Adherence and Retention Facilitators and YMM; Demand creation and support VIAC screening and managing to all eligible HIV positive women; Implementing of community DSD, TLD transition, MMD, TB prevention and VL to patients; Work to complete the retention documentation in all Registers (HTS, Appointment, VL, Defaulter, Index & linkage); Mentor & work with YMM and Adherence and Retention Facilitators in the complete documentation and filing in the patient files; Provide daily/weekly/monthly/ quarterly updates to the District Nurse Coordinator on daily/weekly activities and identify challenges so that they are addressed on time; Work with District Nurse coordinator to prevent attrition among ART patients by daily compiling of missed appointment lists in collaboration with the data clerk; Support retention activities such as effective referral through use of referral slips and defaulter identification and tracking registers; Plan activities for timely (daily) tracking of missed appointment; Support routine data collection, weekly and monthly timely reporting; Any other duties and responsibilities as tasked by the District Nurse Coordinator.

Qualifications
Minimum of a relevant, MOHCC Counselling course, diploma or degree; HIV Rapid testing qualification a distinct advantage; Previous mobilizing, HIV testing and M&E experience would be an added advantage; Good standing with the community being served is a must’ Good knowledge of English and report writing skills required.
